Make-A-Wish Hawaii

Along with the generous donation from Console Connect, we have the opportunity to contribute to this fundraiser to help grant wishes for children with critical illnesses. Together, we can do it.
Wish kids are some of the bravest and sweetest kids you'll ever know. They wish to work in a pickle factory, become a superhero, go snorkeling, give their baseball team new uniforms, and push all of the buttons in a tall building's elevator. Their innocence and unexpected wisdom stays with you.
Make-A-Wish grants thousands of unique wishes each year. A lot, yes. But that's only 50 percent of eligible kids. We are fundraising so more deserving kids and their families can know the happiness, relief, and renewal a wish can bring.

A digital signature is required of each runner or parent/legal guardian for minor.
I (and any of my representatives) understand this event may include dangerous potential to cause the risk of serious injuries and sometimes death. I agree that it is my sole responsibility to determine whether I am sufficiently fit and healthy enough to participate in the Event, that I am responsible for my own safety and well being at all times and under all circumstances while at the Event site. I will agree to any decision made by the Event authorities, in order to carry out the race safely. I understand the climate of high temperature, humidity, traffics, road conditions, bridge conditions and any hazards associated with this race. I will exempt/release any and all liabilities regards to the Event organization, Event sponsors, Event authorities and personal in connection to this race event for any accidents or damages from event related activities, including without limitation, the potential for serious bodily injury, sickness and disease, permanent disability, paralysis and loss of life; loss of or damage to equipment/property; exposure to extreme conditions and circumstances; contact with other participants, spectators, animals or other natural or man-made objects; dangers arising from adverse weather conditions; imperfect course or track conditions; land, water and surface hazards; equipment failure; inadequate safety measures; participants of varying skill levels; situations beyond the immediate control of the Event organizers; and other undefined, not readily foreseeable and presently unknown risks and dangers. I understand that the participation fee is non-refundable and the Event organization and authorities involving in this event has the right to make any changes including time, schedule and cancellation without refund, in any case of emergencies such as natural disaster and any other emergencies. 
I consent and will give all authorities and copyright to the Event organization for the use of photography, motion pictures, recordings or any other record of this event for the legitimate purposes use in TV, Radio, Newspaper, Magazines, Internet and any Media publishing.
I also waive/release any responsibilities regards to the Event organization for any lost and theft during the event.
I hereby agree, understand, and acknowledge to the competition rules, regulation and covenant agreement at the time of completing the application form.

I have read this agreement and fully understand its terms. I understand that I have given up substantial rights by signing it, and sign in it freely and voluntarily without any inducement.
